Community Health Worker Program


CAMBA’s Community Health Worker Program started in 2001 and builds upon the Healthy Families Program.  The program serves Brooklyn’s 11226 and 11203 zip codes and provides pregnant women and parenting families with case management services focused on improved mother-baby health outcomes.  It is targeted to the hardest-to-reach populations in the community---the poor and medically underserved.  Services may be offered on a short- or long-term basis and may last until the child is two years old.  Community Health Worker Program currently serves 140 families per year.
