The Brooklyn Partnership to Drive Down Diabetes (BP3D) has programming that focuses on diabetes and obesity management and prevention, including interventions that promote healthy eating and physical activity.
BP3D provides case management for individuals with diabetes as well as empowering diabetes self-management courses. Our program also offers free services to promote nutrition and physical activity through partnerships with Start Your Engine and Body Sculpt of NYC.
On September 1, 2011, we launched “Our Health is in Our Hands,” a community-wide social marketing campaign. The campaign is part of our efforts to address risk factors for diabetes and obesity in our communities. We encourage you to explore this page, to learn about healthy options already available in Brooklyn and to learn ways to advocate for even more healthy options! We will be adding content on an on-going basis so please check back often.

Since the launch of BP3D, we have also formed the BP3D Community Coalition. The BP3D Community Coalition consists of over 40 members, many of whom represent our core partners: CAMBA, GBHC (Greater Brooklyn Health Coalition), CHN (Community Health Network), BKDPHO (Brooklyn District Public Health Office)---as well as supplemental stakeholders, government representatives, representatives from various Brooklyn-Based community organizations and low-income residents.
Our coalition is actively finding ways to address factors that influence health disparities in low-income communities of color. In the beginning of 2011, we conducted a Community Needs Assessment of Central Brooklyn and East New York to inform “Our Health is in Our Hands” and to help enhance BP3D’s services to the community. Later this year, we will be conducting a Provider Needs Assessment to get feedback on how we can help providers better serve diabetics in Brooklyn. In 2012, we will be creating and implementing a strategic plan on how to strengthen diabetes and obesity prevention efforts in Brooklyn.
