Brooklyn Prenatal Care Consortium Comprehensive Care Outreach’s (BPCC) sub-contract with CAMBA began in 2003 and its mission is to provide a comprehensive continuum of care for at-risk Brooklyn women of child-bearing age in the following targeted communities: Bedford Stuyvesant, East Flatbush, Flatbush, East New York, Fort Greene, Williamsburg and Sunset Park. BPCC has sub-contract agreements with CAMBA’s Prevention Services for Women, Healthy Families and Community Health Worker Programs to outreach to at-risk women, to provide case management services and to refer women who are pregnant and/or HIV positive and not in care to a continuum of services. This continuum includes a whole range of services including primary prenatal and perinatal medical care, short and long-term case management, substance abuse and detoxification services, and HIV specialized care to name a few. These services are delivered in a culturally and linguistically appropriate setting with a high-intensive engagement strategy. The long-term goal is the reduction of adverse birth outcomes, including perinatal HIV transmission.
