Brooklyn Prenatal Care Consortium Comprehensive Care Outreach's (BPCC) sub-contract with CAMBA began in 2003 and its mission is to provide a comprehensive continuum of care for at-risk Brooklyn women of child-bearing age in the following targeted communities: Bedford-Stuyvesant, East Flatbush, East New York, Flatbush, Fort Greene, Williamsburg and Sunset Park. BPCC has sub-contract agreements with CAMBA's Prevention Services for Women, Healthy Families and Community Health Worker Programs to outreach to at-risk women, to provide case management services and to refer women who are pregnant and/or HIV-positive and not in care to a continuum of services. This continuum includes a whole range of services including primary prenatal and perinatal medical care, short and long-term case management, substance abuse and detoxification services, and HIV specialized care to name a few. These services are delivered in a culturally and linguistically appropriate setting with a high-intensive engagement strategy. The long-term goal is the reduction of adverse birth outcomes, including perinatal HIV transmission.
Community Action for Prenatal Care (CAPC) is an intense outreach initiative for women who are at risk for substance abuse and HIV infection and have late or no prenatal care who live in zip codes 11203, 11226 and 11233. These women are then referred to case management services such as: COBRA, Healthy Families and Community Health Worker Pograms and for the appropriate healthcare services.
