Healthy Families Initiative


On July 1, 2009, through funding from NYC DYCD, short-term case management services are provided to pregnant families or families with children under the age of 18 who reside in zip codes: 11210, 11218, 11226, 11230 and 11233. The case manager helps connect families to resources that will allow them to reach their goals and attain maximum health. The case manager provides information about such topics as: child development, health insurance and family planning through workshops and home visits. The program services 80 families.
