The Consolidated Omnibus Budget Reconciliation Act (COBRA) started in 1996 and provides intensive case management services for HIV/AIDS clients and their families. The program is composed of three-person teams: a case manager, case manager technician and community follow-up worker. The three team members work with clients and their families to stabilize living situations (medical, housing, benefits, etc.). All services are billed through Medicaid. Our Community Follow-up Program currently serves 190 clients.
