Reconciliation Act (COBRA), also known as the Community Follow-up Program


This program provides intensive case management services for HIV-positive clients and their families.

Started in 1996, the Community Follow-up Program currently serves 200 clients. We deliver services using three-person teams comprised of a case manager, a case management technician and a community follow-up worker.  The teams work with clients and their families to stabilize their living situations by addressing needs such as medical care, housing, and access to public benefits. All services are billed through Medicaid.
