HIV/AIDS & HEALTH-RELATED PROGRAMS

Case Management Programs

Consolidated Omnibus Budget Reconciliation Act (COBRA) Also known as the Community Follow-up Program (CFP): This program provides intensive case management services for HIV/AIDS clients and their families. The program is composed of teams of three people:  case manager, case manager technician and community follow-up worker. The three team members work with clients and their families in order to stabilize their living situation (medical, housing, benefits, etc.)  All services are billed to Medicaid.

Grant Funded Case Management: Intensive case management services are provided to HIV positive clients who do not qualify for services under COBRA due to their Medicaid ineligibility.  Income exceeding the Medicaid guidelines or the lack of legal status in the United States can make someone ineligible for Medicaid. Services include advocacy for medical benefits, entitlements, access to housing etc.

HIV Prevention Case Management (PCM): This program provides prevention case management services and educational workshops to both HIV positive and HIV negative individuals about reducing the risk of transmitting and contracting HIV. In addition, the program provides community outreach and dissemination of risk reduction information.

Educational & Supportive Programs

Living Independently through Education and Support (LITES): This program provides Independent Living Skills Development through a 20 credit workshop curriculum.  The program starts and completes the workshop curriculum each month.  In addition, the program has a Nutritionist on staff who conducts group workshops and individual counseling sessions on nutritional topics and develops independent individual plans.  The program also offers structured socialization / recreational activities and a support group for its male clients.

HIV Prevention for Positives: Through a subcontract with Brooklyn Hospital Center, Prevention for Positives provides harm reduction skills building, partner notification counseling, and prevention case management on-site at the hospital’s Program for AIDS Treatment and Health (PATH Center). In addition, staff conducts regular educational workshops and group sessions aimed at reducing clients’ risk of re-infection and transmission of HIV.

HIV Prevention Services for Women: This program primarily reaches out to women at high risk for HIV infection and women living with HIV to reduce their risk of acquiring or transmitting HIV.  Through trained Peer Counselors, the program disseminates prevention information and engages women at risk.  The program provides educational workshops and risk reduction interventions through group and one-on-one meetings.  Participants also receive supportive services and referrals to medical and appropriate social services.

Supportive Counseling & Family Stabilization: This program provides individual and family supportive counseling to families and single individuals in need of this important service.  One member must be HIV positive.  The ultimate goals for the program are to stabilize the family unit through counseling and support services and to ensure that the HIV positive individual is connected to and maintains their primary health care.  Other supportive services are provided in the form of low-threshold case management services, support group and structured group socialization and educational activities for children.

Women Supportive Service’s (WSS):  This program provides social/recreational activities, emergency assistance, weekly support groups and individual counseling to women living with HIV/AIDS and their families.

East New York/Brownsville HIV Care Network

CAMBA is the lead agency for this HIV Care Network.  The Network is designed to disseminate information on HIV/AIDS services and trends to members of the East New York / Brownsville communities and to organizations that provide services to these specific communities and to Brooklyn residents in general.  The network accomplishes its goal through the provision of educational meetings, workshops, and events all designed to increase awareness around the availability of HIV services, funding opportunities and the prevention of the further transmission of HIV.

Health Programs

Acupuncture and Holistic Medicine: This program provides auricular acupuncture to HIV positive individuals, particularly clients who are active substance abusers or in recovery. Family members of the HIV infected individual may also receive treatment. Acupuncture is utilized in conjunction with both individual and group counseling.

Beyond Hunger Emergency Food Pantry: CAMBA has operated an emergency food pantry for over 10 years. The Food Pantry distributes a three-day supply of nutritional food to CAMBA clients as well as clients referred by churches and other agencies. Services also include review of public benefits and assistance with applications, money management, nutritional education, referrals to social service programs and health care providers, etc.

Greater Southern Brooklyn Health Coalition (GSBHC): CAMBA houses the Coalistion and provides daily administrative and fiscal oversight.  Formed in 1997, GSBHC was a project of the Jewish Community Relations Council’s department of inter-group relations and community concerns.  As a 501(c) (3) agency, GSBHC is comprised of over 60 agencies (community-based-organizations, hospitals, health insurance companies, home care agencies, nursing facilities, and mental health agencies) that span the geographic and ethnic diversity of Brooklyn.  GSBHC focuses on coordinating and building relationships with agencies (public and private, city and state) within Brooklyn and throughout the Metropolitan New York area to respond to the public health needs of the residents of Brooklyn by serving as a voice, fundraising and developing programs to address the needs of the community, and to provide technical assistance to its member agencies.
