Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive. We take a comprehensive approach by offering more than 160 integrated programs in: Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach more than 65,000 individuals and families each year at our 90 locations, including 10,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are in living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.
CAMBA's Breathe Well program is designed to address the widespread incidence of asthma in low-income communities of color in Brooklyn. The program seeks to improve the quality of life for people with asthma and reduce the cost of asthma treatment through the use of Asthma Health Coaches. These Coaches will conduct home visits to assess for conditions that negatively affect individuals with asthma; provide education around best practices on asthma; provide advocacy and connection to resources to remediate conditions that exacerbate asthma; and work with clients' providers on developing asthma action plans that include medical treatment and control of environmental triggers. By providing the tools for successful self-management, CAMBA aims to ensure that people with asthma can lead healthy, active lives.

What The Health Coach Does:
The person filling this position is expected, under general direction, to: (1) provide individual counseling services to assist individuals to understanding health literacy; (2) counsel clients in dealing with such issues as: behavior, educational progress, family dynamics, mental and physical health, housing, personal finances, substance abuse, mental and/or physical trauma.
