CAMBA's Health Home, an innovative health care management program focuses on improving the health outcomes for individuals with severe mental illness and other complex chronic illnesses. Staff uses cutting-edge health information technology and a unified plan of care to ensure all clients connect to and maintain primary medical care, prevent hospital stays and reduce the length of hospitalizations.
Essential Duties and Responsibilities:
The person filling this position will oversee a team of Supervisors and Care Coordinators servicing clients enrolled in CAMBA's Health Home Program, including at least one specialized team servicing clients enrolled in Health and Recovery Plans (HARP.) The person filling this position is expected, under general direction, to ensure the smooth day-to-day running, coordination and supervision of all program-related staff and services in accordance with all program goals, targets, and performance outcomes and all CAMBA and program policies, procedures, and protocols; including but not limited to the following:
