*Program Manager (Clinical), Health Home

CAMBA's Health Home, an innovative health care management program focuses on improving the health outcomes for individuals with severe mental illness and other complex chronic illnesses.  Staff uses cutting-edge health information technology and a unified plan of care to ensure all clients connect to and maintain primary medical care, prevent hospital stays and reduce the length of hospitalizations.

Essential Duties and Responsibilities: 

  • Liaison with medical staff and psychiatric providers to ensure appropriate service delivery for clients.
  • Implement a system to ensure that clients maintain their medication compliance on a weekly basis.
  • Oversee all program operations and manage direct reporting program staff and their subordinates

MINIMUM EDUCATION/EXPERIENCE REQUIRED:

Master's degree and five years of applicable experience, and/or equivalent experience. (A license is required of these Master's Degrees: Social Work (LMSW or LCSW), Mental Health Counseling, Counseling/Guidance, Art Therapy).

OTHER REQUIREMENTS:

  • Knowledge about, understanding of, and ability to work closely with, persons with chronic illnesses.
  • Experience with the use of technology and the integration of technology into program workflow.
  • Experience with community outreach and/or sales.  
  • Bi-lingual English with any one of the following languages: Spanish, Russian, Haitian-Creole, Chinese (Mandarin or Mandarin/Cantonese) preferred.
  • Some evening/weekend availability may be required.
