Case Manager, CAMBA’s Maternal and Infant Community Health Collaborative

CAMBA's Maternal and Infant Community Health Collaborative (MICHC):  Through a grant from the New York State Department of Health, CHW seeks to address maternal and infant health behavior, supports and service systems across three key life stages: preconception, prenatal/postpartum, and interconception. CHW provides high need women and infants with services focused on improved maternal and infant health outcomes to the hardest-to-reach populations in the 11203, 11226, 11233 and 11212 communities.

Essential Duties and Responsibilities:  The person filling this position is expected, under close supervision, to: (1) aid individuals and/or families, in accessing and enrolling in needed preventive and primary health care services;  

(2) Provide assistance and or/referrals to obtain essential support services as appropriate to specific life stage; (3)  locate and make use of appropriate community resources  that promote and support maintaining healthy behaviors. for participants; (4) Conduct basic health assessments, assist families to identify and address risk factors through timely and coordinated counseling, management, referral and follow-up.​

MINIMUM EDUCATION/EXPERIENCE REQUIRED:

Bachelor's Degree or High School diploma/GED and 4 years of applicable experience and/or equivalent experience.
