CAMBA’s DYCD Healthy Families/NDA 16 Program provides case management and home visiting services through the Healthy Families Initiative and is offered to families in Community District (CD) 16. The contract from The Department of Youth and Community Development (DYCD), allows the program to provide local families with services focused on family support, and improved health outcomes targeting the hardest-to-reach populations in the zip code 11212 community. Using a case management approach, it addresses the particular needs of each participant.
Essential Duties and Responsibilities: The person filling this position is expected to: (1) aid individuals and/or families requiring social service assistance; (2) interview and evaluate applicants for services, formulate service plans and short or long-term goals, and aid clients to implement service plans; (3) locate and make use of appropriate community resources for clients; (4) evaluate actual living conditions of participants through home visits.
MINIMUM EDUCATION/EXPERIENCE REQUIRED:
High School diploma or GED and 4 years of experience and/or equivalent experience.
