CAMBA's DYCD Healthy Families/NDA 14 Program: Through a grant from the New York State Department of Health, CDHFP provides local pregnant and parenting families with case management services focused on improved health outcomes and targeted to the hardest-to-reach populations in the 11210, 11218, 11225 and 11226 communities. Through a contract from DYCD case management services through the Healthy Families Initiative will be offered to families in Community District (CD) 14.
Essential Duties and Responsibilities: The person filling this position is expected to: (1) aid individuals and/or families requiring social service assistance; (2) interview and evaluate applicants for services, formulate service plans and short or long-term goals, and aid clients to implement service plans; (3) locate and make use of appropriate community resources for clients; (4) evaluate actual living conditions of participants through home visits.
MINIMUM EDUCATION/EXPERIENCE REQUIRED:
High School diploma or GED and 4 years of experience and/or equivalent experience.
