Case Manager I (Assessment Specialist), Homelessness Prevention Initiative (HomeBase) - Staten Island
CAMBA's HomeBase Program is seeking an Assessment Specialist!
CAMBA's HomeBase Program provides case management and essential services to at-risk individuals and families in Brooklyn and Staten Island to prevent homelessness. Individuals and families receive services that help them overcome problems with public benefits, housing, education, health care and other issues that could impede their ability to maintain stable households.
Key responsibilities include:
Aid individuals and/or families requiring social service assistance by interviewing, reviewing required documentation and evaluating applicants for services while maintaining courteous, professional relationships and confidentiality
Maintain thorough and up to date client files, input client data and client progress information into automated database, present cases to Program Manager for service plan formulation and provide all required information for weekly/monthly/quarterly/annual reports
Assist clients in identifying and locating community resources, providing referrals and assisting clients in completing applications for benefits and entitlements as well as encouraging clients to advocate for themselves to move them toward self-sufficiency
Reach out and market the program to the community in order to recruit clients
Qualifications sought:
Bachelor's degree and/or equivalent experience.
Proficient experience using Microsoft Office software and knowledge of databases.
Knowledge of, and experience with, New York City Housing and Public Assistance Systems.
