*Program Manager (Quality Assurance Specialist), Health Homes

CAMBA's Health Home, an innovative health care management program focuses on improving the health outcomes for individuals with severe mental illness and other complex chronic illnesses.  Staff uses cutting-edge health information technology and a unified plan of care to ensure all clients connect to and maintain primary medical care, prevent hospital stays and reduce the length of hospitalizations.

Essential Duties and Responsibilities:  The person filling this position is expected, under general direction, to ensure the smooth day-to-day running, coordination and supervision of all program-related staff and services in accordance with all program goals, targets, and performance outcomes and all CAMBA and program policies, procedures, and protocols; including the following: 

  • Develop and implement Quality Assurance measures to ensure quality service delivery to clients.
  • Ensure that all client files and program files are kept in compliance with CAMBA's and funder's standards.
  • Report trends on compliance and performance issues to supervisory and managerial staff
  • Conduct client and program progress reviews with staff.
  • Prepare, review and ensure the accuracy and timeliness of, AIRS billing documents, contract reports and statistical information for both CAMBA management and funder use.
  • Analyze program and demographic data to make recommendations for programmatic improvements.
  • Recommend trainings for supervisory staff based on trends identified in chart reviews
  • Confer with the Senior Program Manager and/or other CAMBA management staff as needed regarding programmatic and personnel issues.
  • Participate in staff/funder/outside agency meetings as requested.
  • Manage the peer chart review process on a quarterly basis to ensure there is no overlap of charts that are audited
  • Work closely with directors to identify charts needed for monthly supervisory audits
  • Inform Program Director and Vice President of highly deficient charts immediately
  • Follow up on all charts reviewed to verify corrections have been made
  • Communicate with peers in other CAMBA programs on issues related to client progress.
  • Understand all aspects of contract requirements and communicate this understanding to staff.
  • Prepare contract reports and statistical information for both CAMBA management and funder use.
  • Understand and assist with all aspects of contract requirements and communicate this understanding to staff.
  • Conduct client and program progress reviews with staff.
  • Train staff in program responsibilities, processes and procedures and ensure that all staff meets contract requirements in their daily work, as needed.
  • Attend staff/funder/outside agency meetings as requested.

MINIMUM EDUCATION/EXPERIENCE REQUIRED:

Bachelor's degree and three years of applicable experience and/or equivalent experience.

OTHER REQUIREMENTS:

  • Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues.​
